Urgent: Twitter lead gen card works when testing but not when live

Hi Twitter team,

We successfully tested the lead gen card within the ads.twitter settings area - the test details were submitted to the 3rd party database.

Since publishing the card live no submissions are going into the 3rd party database.

Can anyone provide directions - i have followed the supplied instructions exactly.
