Twitter card appearance is inconsistent

I’ve implemented Twitter card markup on my site, been approved for Twitter card use, and can preview the card successfully using https://dev.twitter.com/docs/cards/preview. Nevertheless, the cards only appear sporadically when I link to a particular post. I have examples in my feed of linking to the same post twice–the first time a Twitter card appears with the post, while the second time nothing at all. There has been no change in my link or site syntax between these two events, so I’m puzzled. Thoughts?

@froginthevalley we’re seeing similar behavior with the iPhone app. Most of our tweets from our approved domain acct are showing the photo/summary (nice!). However, other people’s tweets of the same content originating from our domain do not show the photo on the iPhone app though it does show on the desktop webapp.
