We are currently seeing the same trend appearing through the API for the one location at different positions. So for example at the moment for Cape Town (accessed via /1.1/trends/place.json?id=1591691) you get this result with ‘South Africa’ at the 2nd and 7th position (see below). There are a number of other examples of this such as Poland (Has ‘End Date’ listed 4 times!). This problem isn’t replicated in the web client if you canhge your location to Cape Town or Poland though.
