PIN-Auth Page requires info not provided

This just started happening about a week or so ago, when doing PIN auth. The user types their credentials and the next web page shows

“This page requires some information that was not provided. Please return to the site that sent you to this page and try again.”

Watching the Web traffic, the request_token and authorize requests execute as normal, returning 200 each. Then the user redirects to POST https://api.twitter.com/oauth/authorize HTTP/1.1. The response at that page is a PIN number, which I expect. However, the problem occurs when the browser redirects to GET https://api.twitter.com/oauth/authorize HTTP/1.1 and receives a 403. There isn’t anything in my code that explicitly executes these last two POST and Get queries to the authorize endpoint. Here are a couple headers from the bad response:

Did you end up finding a solution to this? I have an application that has worked for the past 2 years, which is now failing with this issue…

Any update for this issue??
The same problem has been happening with iPhone safari (iOS6), but not with Android and iOS7+.
It displays PIN code for a moment and redirects to the url Joe mentioned above.
