Is there an simple example player card that has been approved

Is there an example player card HTML page (the most basic) to look at with a working player card for an .mp4 video it would really help me. I am not a web developer. I keep getting rejected, but my card validates just fine. But is rejected without explanation other than (it doesn’t load).

If your card is validating properly, it was most likely rejected due to a policy violation.
